As a courtesy we will file any insurance claims for you. We do require your portion to be paid at the time of service. The balance is your responsibility whether your insurance company pays the claim or not. In order to bill your insurance company we need you to provide us with all your insurance information, such as your insurance card and the subscriber’s social security number. Our practice is committed to providing the best treatment possible for our patients and we charge what is usual and customary for our area. For you convenience, we are listed as an ODS / Delta Dental Premier Provider. We can bill most insurance carriers. Confirm with the office and your insurance carrier to ensure you understand the costs and responsibilities.

Full payment is due at the time of service. We accept cash, checks, VISA, MasterCard and American Express. For your convenience, we do offer an extended payment plan with an outside financing company. It does require prior credit approval. More information can be found at Care Credit.
